Tirzepatide carries an FDA Boxed Warning for thyroid C-cell tumour risk based on rodent data. It is contraindicated in patients with MTC or MEN2 history.

Acute pancreatitis occurs in 0.6-0.9% of treated patients. Discontinue immediately if severe abdominal pain radiating to the back is confirmed.

Tirzepatide reduces efficacy of oral hormonal contraceptives. Use barrier protection for 4 weeks after initiation and each dose increase.

Delayed gastric emptying creates aspiration risk during anaesthesia. Patients must inform surgical teams before any procedure.
