2.A Mixture, Not a Molecule
Unlike a single synthesized drug, Cerebrolysin is a complex biological mixture. Made only by EVER Neuro Pharma in Austria, it is a proprietary extract from purified porcine brain tissue, produced by enzymatic breakdown of proteins into free amino acids (about 20%) and low-molecular-weight neuropeptides (about 80%).
Mass-spectrometry work has identified fragments of neurotrophic factors including CNTF, GDNF, and IGF-1/IGF-2. This heterogeneity is the whole point of the product, and also its regulatory weakness: it has no single compound identifier and cannot be replicated by another manufacturer.

3.How It Is Proposed to Work
The biological appeal is that the extract mimics neurotrophic factors, the body's own signals that maintain, protect, and restore neurons. It is proposed to push the brain toward self-repair by modulating survival pathways.
Sonic Hedgehog (Shh): promotes neurogenesis and oligodendrogenesis via Ptch and Smo receptors and the Gli complex (animal/in vitro).
PI3K/Akt: BDNF-like activity that supports cell growth, proliferation, and survival while inhibiting apoptosis.
CREB/PGC-1a: linked to reduced neuroinflammation and lower pro-inflammatory cytokines such as IL-1 beta.
It is also reported to attenuate excitotoxicity, reduce reactive oxygen species, suppress caspase-3-driven apoptosis, lower TNF-alpha and NF-kB signalling, and help stabilise the blood-brain barrier.
