Personal or family history of Medullary Thyroid Carcinoma (MTC)
Absolute contraindication. Based on rodent C-cell tumour signal. FDA boxed warning.
Multiple Endocrine Neoplasia syndrome type 2 (MEN 2)
Absolute contraindication due to MTC risk component of MEN2.
Planned pregnancy within 2 months
Potential impact on embryo development. Must discontinue at least 2 months prior to planned pregnancy.
History of acute or chronic pancreatitis
Elevated baseline risk for pancreatic events, compounded by delayed gastric emptying and potential biliary effects.
Stage 4 or 5 chronic kidney disease
GI-related dehydration (nausea, vomiting, diarrhea) can precipitate acute kidney injury in patients with marginal renal function.
Severe hypertriglyceridemia (>500 mg/dL)
Baseline triglycerides above 500 mg/dL carry marked pancreatitis risk, amplified above 1,000 mg/dL.
